Supplemental Approval Form (SAF)

Link to Resource

Revised Date

11-21-2024

Summary

The SAF is used to capture internal approvals for the following:

  1. change in the Principal Investigator or Co-Investigator after the proposal has been submitted or award has been executed.
  2. change in the lead unit (i.e., different 3-digit organization code) after the proposal has been submitted or award has been executed.
  3. a pending Principal Investigator new hire not in Banner.

Helpful Tips

Once the SAF is approved by all parties, SPA Award Management will proceed with submitting the request and obtaining sponsor approval, if required per the terms and conditions of the award. If sponsor approval is required, the award data in myResearch Portal and Banner will not be updated until official sponsor approval is received.  If required, a contractual modification and action by the SPA Negotiation team maybe needed before systems can be updated.

If an approved Co-PI (listed in myResearch award record) moves into a PI role, a SAF is not required.

Submit a separate SAF whenever a change in PI leads to a change in the Lead Unit.
