Summary

Use this template letter to obtain institutional approval on a proposal submission to a Pass through entity where UIUC acts as subawardee. This template was originally developed by the Federal Demonstration Partnership (FDP).  

ONLY the designated AOR (Authorized Organizational Representative) signature from SPA will be valid on this form.  Units may prefill the  Project details and include with their proposal routing to our office and detail our certifications for inquiring Lead collaborators.  SPA will apply the authorized signature upon approval of the proposal.
